How do I request the Care at Home Service?
Care at Home Services can be arranged after an assessment meeting where the person who needs support, other relatives, carers, health and/or social work employees all discuss what the issues are and what services might be needed.
If Care at Home Services are arranged for you a home care diary is provided which gives details of the personal support plan, including the support the Care at Home Service has agreed as being needed. It will also note progress on a daily or weekly basis as appropriate.
