Care at Home Service
Who can use the Care at Home Service?
We give priority to people with serious conditions or circumstances including:
- terminal, chronic or acute conditions or illness
- high risk to physical safety
- inability to carry out daily living tasks
- deterioration of living conditions where a person is at risk of infection
- to prevent a child or young person at risk being accommodated
- to avoid the need to go into a care home because the person's usual care(r) is not available
- breakdown in caring relationship
- support and rehabilitation after discharge from hospital
- vulnerable people preparing to live independently in the community
