| Aims and Objectives | The increase in home care capacity and the introduction of both the integrated community support teams and the hospital at home service will enable community based services to be further developed. In line with Supporting Your Independence, this will also realise efficiency opportunities by adopting a preventative approach to social care needs. |
| Conclusion | The policy will not have a negative impact on any equality group. A co-produced assessment will be undertaken with service users and carers and, based on assessed needs and risks, a support plan will be agreed and implemented. |
