Aims and Objectives | As part of making services more effective an ongoing review of day opportunities for adults attending integrated community facilities (Lifestyles) will continue. By further developing the person centered approach to service delivery, improved outcomes for individuals will be secured and efficiency savings realised. These discussions have lead to consideration of appropriate models of support for service users such as: • Day Centre Model – ‘the hub – service provision within the Lifestyle Building’; • Support Model – ‘supporting service users to access community resources’; and • Facilitative Model – ‘progressing independent access to community resources’. |
Conclusion | Day Opportunities for Adults with Learning Disabilities will continue to be reviewed in line with new locality arrangements for health and social care integration, the changing needs of service users, new models of support and the choice and control service users can have with Self Directed Support. |
