Health and Social Care Integration
Health and social care services in South Lanarkshire work together providing more joined‑up care. This means you can access support in a simpler way, with services planned around your needs and delivered as close to home as possible. The South Lanarkshire Health and Social Care Partnership brings together staff from NHS Lanarkshire and South Lanarkshire Council to provide this support.
How integration works
Since April 2016, health and social care for adults has been planned and funded by the South Lanarkshire Integration Joint Board. The Board is responsible for deciding how services are designed and delivered, and for making sure they improve people’s health and wellbeing.
The partnership focuses on care that is preventative, personalised and coordinated. We work with residents, carers, local groups and professionals to shape services that help people live well, stay independent and get the right support at the right time.
Our commitment to equality
We must follow the Equality Act 2010, which means we must treat everyone fairly and promote equal access to services. As part of this duty, we publish:
- a report on how we are mainstreaming equality
- a set of equality outcomes that guide our work
Our Strategic Commissioning Plan is built around these equality outcomes. It sets out how we will reduce health inequalities, improve access to support and make sure all services are delivered in an open and inclusive way.
Reducing health inequalities
Health inequalities can be linked to housing, education, employment and access to care. We work with NHS Lanarkshire, South Lanarkshire Council and wider community partners to address these issues. This includes supporting early intervention, improving local services and assessing the impact of our policies so that everyone has an equal chance to live well.
